1.TERMS AND CONDITION By signing up through the online vaccination scheduler, you agree to the following terms and conditions: 1.1 The vaccination activity is permitted only for adults aged 18 years old and above. \ 1.2 Medical screening by a qualified physician shall be required prior to the administration of the vaccine. This will include taking of your medical history. You commit to provide truthful responses to the patient consent form, health status questionnaires, and other registration documents. Giving of false information is punishable under Republic Act No. 11332. 1.3 The appointment schedule assures you of your slot in our vaccination-capable stores. However, as the vaccination will be subject to medical screening, we cannot guarantee that the vaccine will be administered to you. 1.4 Upon clearance by the physician, you shall settle the applicable service fee for the vaccine to be administered. 1.5 You will follow the public health safety protocols observed and implemented by the Watsons store personnel. You may refer to our health protocols in this link. 1.6 You will also be subject to the standard terms and condition for vaccination procedure of Watsons including but not limited to the monitoring and reporting of adverse events following immunization. You may refer to our vaccination procedures in this link. 1.7 Should you wish to purchase the vaccine vials only, you will be required to submit a prescription for the vaccine administration, demonstrate ability to maintain the cold chain requirement (e.g. cooler) and sign the relevant waiver and release form. 1.8 All processes, forms and documents indicated and/or referred or submitted to Watsons Personal Care Stores (Philippines), Inc. or FamilyHealth & Beauty Corp. will be processed in accordance with the Privacy Statement found in this link. 1.9 You acknowledge and agree that we may have the legal obligation to report to or notify relevant health authorities of any adverse event experienced immediately or within such period as may be provided by applicable law. In this regard, you undertake to promptly report and cooperate to provide any information that may be required for such reporting or notification of adverse event. 1.10 You can order your vaccines via the Watsons Online Vaccination scheduler and get vaccinated from your selected store at your selected schedule 1.11 There is no minimum purchase requirement and available with credit, debit and e-wallet option. 1.12 You will receive an email notification confirming your vaccination schedule and online payment. In case you will not be able to personally use your vaccination order, please make sure to prepare an authorization letter and proof of authorized person’s identity (e.g. any government ID) of the person who will use your vaccination order in your place. 2. Price and Payment 2.1 The price of any Products will be as quoted on the Site from time to time, except in cases of obvious error. 2.2 These prices may include applicable discounts and local taxes. To apply for special discounts and tax exemptions for senior citizens, persons with disabilities, national athletes and coaches and other similar special category discounts, please consider availing of our products and service via Watsons store near you. 2.3 Payment can be made by most debit or credit cards and other payment methods acceptable to us, by completing the relevant details on the “Payment” page. We shall not be liable for any credit card or other payment fraud. 2.4 All product prices listed are in Philippine Peso (PHP). 2.5 Full payment in respect of an order must be made upon the placing of the order. If your payment cannot be processed, the order will be not be accepted and you are advised to contact your card issuer to resolve any problem concerning the use of your credit card in order to continue with the order. 2.6 We reserve the right to change the payment terms for any order without prior notice. 2.7 If payment for any order has already been made but the order is not accepted by us, you will be entitled to a refund, depending on the period of time your financial institution needs to arrange the refund. We shall not be liable with respect to any loss, damage, cost or expense that you or any person may incur as a result of any delay in your financial institution processing the said refund. 3. CANCELLATION POLICY 3.1 All cancellations must be made prior to checkout. No cancellations will be accepted after checkout has been completed. 3.2 Orders may be cancelled by Watsons due to the following reasons: a. If the order has been evaluated as a potential case of fraud; b. Other cases when order cannot be fulfilled. 3.3 In the event that Watsons cancels an order, it shall be in full, not partially/per item. 3.4 Customers will be duly notified in the event that an order has been cancelled. 3.5 For cancelled orders that were paid through credit card, refund shall be processed through credit card payment reversal following Watsons’ standard processing period. 4.REFUND POLICY 4.1 This Refund policy is only applicable for Watsons Philippines Vaccination Scheduler orders. 4.2 Requests will only be entertained within seven (7) days of receipt of the product. 4.3 Request may be made for the following reasons: a. If the products is in damaged or defective condition; b. If the product is near expiry or expired; or c. If the product is incorrect or different from the order placed, or different from how it is described on the website. 4.3 Watsons Customer Service representative may request the submission of electronic files, including photos, receipt, etc. to support the review process. 4.4 A notification will be sent to the Customer once the return and refund request has been approved. 4.5 The manner of refund will depend on the mode of payment originally made by the Customer: 4.6 The Company reserves the right to reject any exchange or refund for opened or used product in accordance with DTI rules and regulations. We shall not be liable to you for any losses, liabilities, costs, damages, charges or expenses as a result.
